    1. Muskegon County Fair -- Horse Registration Form

      Form must be submitted before June Horse Committee Meeting

    2. Age as of January 1st current fair year.

    3. HORSE INFORMATION

    4. If owner is someone other than Exhibitor or Parent, Lease Agreement must be submitted online as well as a hard copy turned in at horse meeting.

    5. If applicable, indicate size of pony in inches.

    6. Registration Information*

    8. If registered, indicate Breed Registration Number.

    9. Current picture of horse/pony is required. Printed copies of photos must be turned in at June horse meeting.

    10. Negative Coggins within the last 12 months must be submitted. In addition to submitting online, a printed copy of the coggins must be turned in at the June horse meeting.

    11. EXHIBITOR CONSENT

      Exhibitor's Parents and Organization Leader attest that the Exhibitor is trained and qualified to compete in the following events and has proven the ability to control the horse while competing.
